I am admittedly overjoyed that you decided to follow up on our previous correspondence. This is surely the beginning of an exceptional relationship. Here at the Four Corners Expeditionary Corps, we are more of a family than a mere organization, and many beyond myself are eager to have you enter our fold. All that is required now, is for you to fill out the dossier form attached and submit it to our offices via fax or email.
It shouldn't require iteration, but for legal purposes, you will be subjected to background checks and vetting. Any details that you decide to omit will be illuminated. Such matters are confidential, of course, but I always encourage future employees to refrain from obfuscating pertinent details, distasteful as they may be. Omission always looks worse than confession, after all.

Skill Set: Where does your character excel? Are they a specialist or a jack-of-trades? Be somewhat reasonable.
Edges: Something that sets you apart from the chaff. An Edge requires a Flaw.
Flaws: What holds you back? Addiction? A limp? A crippling phobia of snakes?
Psychological Profile: Personality, quirks, existing traumas or conditions, and so forth.
History: Biographical details
