History (Detail is good here): Matthias Callaghan was born to a wealthy family in Boston, and was groomed for the medical profession. With the driving of his family and his natural intelligence, Matthias entered and excelled in medical school. There, he met and married a nurse named Margaret. After graduating at the age of 24, they decided to leave his parent's influence behind and go out to the frontier, where they could find adventure and their own path.
After several stops along the way, Matthias and Margaret found a small mining town in need of a doctor. Setting up shop, they was continually surprised at what they took for granted back home were luxuries or just not available out west. It took some adjusting, but the young couple soon felt at home in the small town.
A year after the Callaghan's arrival, the local sheriff was critically injured in a fire fight with outlaws. While Matthias and his wife tried to save the sheriff, the outlaws returned to finish the job. Matthias tried to get them to leave peacefully, saying that all life was sacred. He was laughed at and shot, causing him to fall unconscious. When he awoke, he found his wife and patient were dead, killed by the outlaws.
With his wife dead and his idealism shattered, Matthias sank into a deep depression, spending the next 4 months drunk. He probably would have kept on this path if he hadn't started a bar fight with a retired army officer, which he lost. The officer, named Stevens, sobered Matthias up and got him to face the world again.
Coming to terms with some of the harsh facts of life, Matthias resolved to continue to help people, but realized that relying on words to stop those determined for violence wasn't going to save those he was trying to heal. His first step down this path was learning how to knife fight. With his steady hands, he proved to be quite good with a knife, and could peg a moving target from a dozen yards away easily. While he preferred this mode of violence, he heeded Stevens' suggestion that sometimes knives weren't enough and learned how to use a gun as well.
For the next 16 years, Matthias roamed the West as a traveling doctor. He got a reputation not only for his medical skill, but his tendency to leave the bodies of those who tried to harm his patients out as a warning for everyone to see. Well, those he killed outright. Those who survived his reaction to them would be treated by him, then handed over to what ever law was around.
Over the last year, Matthias went back east to attend his parents's funeral. There, he found where he had grown up and who he had grown up with felt alien to him, and him to them. After paying his respects and saying goodbyes to remaining family, Mathias headed back out West. Back home.
