question about username change!

so i'm aware that you can change your username once every year,


but i'm a little confused by the ''note to moderator'' part of the username changing page.





is it something you have to fill out in order to get your username changed?


and if so, what would be an example of something you'd put there?

You don't have to fill it in. Sometimes, people like to give a reason about why they're changing their name for record purposes, but it's really not important to fill out.
