Rank 1 is considered on a whole other level then even rank 2. Single Digit ranks MUST have a Demon or Spirit contract.

Witch or Sage type: offensive, defensive and special. 
Primary Magic: Every witch can perform many different spells, even spells of other types if they wish, but your primary type of magic is the one you focus on, your spells of this type would be more powerful than if someone else were to use your type of magic. your magic type can be vague.

Consumed Witch appearance: What does your consumed witch look like (i would just use some picture from Claymore, or a demon picture or something)
Demon or Spirit contract: "Each" Witch and (most) Sage makes a contract with either a Demon or a Spirit. Demons are of demonic in nature, and primarily focus on destruction and are primarily offensive, whereas Spirits are spiritual in nature, and primarily focus on being different and are primarily defensive and special in nature. (This is not always the case, you can have a defensive Demon if you want). Not all witches and sages make contracts, but if you join the Coven it is required you make a contract if you are a witch (not so for sages).

At first glance, Eienoha appears perfect. Very attractive, easy going, intelligent, all round fun person to be around. But then someone goes to far, and his other side is shown. He is very, very easily angered and tends to get into fights with people once angered. He can go off at the slightest provocation, destroying the cause. He can go off like a switch, taking out lots of people, only to go back to normal. He has virtually no fears, almost nothing to hide. He is absolutely terrified of...

He likes to be a sage, work with witches and all, but he hates how people like to get him angry and see who'll beat him. He hates annoying people, but can learn to tolorate it. He loves anything sweet, let it be food or people's personality. But he loves to fist fight. He doesn't however, like to get into fights with girls. It's awkward, once you win and look at what you just did. Especially since he has trouble holding back. So many things play into his personality it's practically impossible to name them all. There are just too many Variables that factor in.

History:  Eienoha was born to a wealthy family of 5. Always inside, Eienoha was taught that peace is the best option, no matter what. He didn't like it, but what could he do? His early time was filled on mental ability, with limited time to spare. That time was spent, with two violence loving brothers, that was not to be. Forced into fist fights, He was forcedbto get tough or end up unconscious. So he did, fighting everyone that got him angry, and lost most of them. A child had no hope of actually beating up a fully grown man. Yet he kept on, becoming faster and stronger,  and more angry. He grew to hate everyone in the town, wishing them dead. He continued to fight and win brawls with people who angered him, getting hate from the town. It was one day, where he stood on the roof of his parent's home and shouted, wishing them all dead. The next day, he awoke to find the whole town burning, in the process of being ransacked by bandits. His family was butchered right in front of him, him left to die. But he felt nothing. Once they left, he simply pulled up a chair, wiped off a table and began to eat breakfast. He acted as though nothing happened, until a collective sage came to investigate only finding him, pouting two cups of water. Offering him one, he smiled and asked if he could join, which was accepted.

The next years were spent honing a hand tochand skill, until he sought more power. He ventured out back to the burned town, going to his former room. There, a demon lord sat, waiting for him. The demon had been with him since birth, but it was uninteresting to give him power from the start. A deal was proposed. Skill he previously could never reach, for both his arms. He shrugged, and the demon smiled, miving into his head. The demon cut his arms off and replaced then with new, metal, claw fingered gauntlets...

that definitely held more promise then the previous ones. He came back, now quickly taking ranks, making fourth until he got bored and stopped, satisfied for now.
