More of a personal issue than something objectively awful, but it fits the criteria of a "confession": I feel responsible for my friend and I getting into a nasty car accident over the summer. We were driving down a highway - she was driving, I was in the passenger seat. I saw the car stopped ahead of us and didn't say anything, assuming my friend saw it and would slow down. She did not. (We walked away virtually unscathed, by the way - just bruises, abrasions, a fear of cars, and an expensive flight to reschedule).
I had ample time to warn her. If I'd said something, I would've saved both of us, more so her I imagine, a lot of problems.
