The character sheet should go something along the lines of:
Name: (A name)
Age: (Five-year olds may not be the best choice here...)
Appearance: (If it's a picture, please be sure to describe any differences. Likewise, be descriptive if you choose to go without pictures)
Personality: (What's your character like if you were to meet them in real life?)
History: (No need for an epic poem or history book, important parts of their life that shaped them. Negative experiences usually build character, but don't forget about positive ones!)
Strengths/Weaknesses: (Try not to have too many OHKO's)
Keyblade Design: (Go crazy! Drawing, picture, pure description. Just make sure other people can visualize it)
