The raging lunatic is a common mistake roleplayers make when creating insane characters. I am not exactly an expert, but I have been writing insane characters for a while and they do require some more nuance than that.

You're right. Weaknesses are physical traits. Bad personality traits are known as flaws, not weaknesses.
Example: Cold is a weakness. Being half-insane is a flaw.

If your character looks like a twelve year old then she better be a twelve year old. Even an adult trapped in a twelve-year-old body would be preferable to what is essentially a sexualized child.
His situation is competely different as he lives in a world where almost everyone in it would be considered OP. Plus the writers see fit to actually allow him to be defeated very once in a while by more powerful opponents.
OP as in extremely overpowered, in other words yes. Can't be beaten easily or at all because the character is either completely invinicible or so bloody powerful that it's impossible to last long enough to beat them.
