This would definitely vary, but if your character is known to associate with anyone with a criminal/deviant history or if they are suspected to be doing any activities that can’t be accounted for! Also if there’s any reason to believe your character is in contact with hostile foreign nations...

And to answer some of the above questions: I think for the ears and tail, their tech wouldn’t have yet developed to the point they can assign those kinds of genes and have them be hereditary, so it would have to be a procedure she got during her lifetime. Although they do live over 100 years...
