To be honest, at first I wondered if you meant insane as in crazy or insane as in super badass. Glad it was cleared up by other replies.
In my eyes, the most insane characters aren't measured by how many laughs they crack or how simply evils their plans are, but by how their insanity twists their view without them realizing, and the lengths to which they go on their warped path, how affected they are by their loss of sense. So here's a quick top 5, with spoilers beneath explaining (and these spoilers do actually containg some heavy spoilers for a chunk of them). As a quick sidenote, I purposely only had one character per series, to avoid having to name like half the cast of some series
1. Kaneki Ken from Tokyo Ghoul
2.Jabami Yumeko from Kakegurui
3.Uryuu Ryuunosuke from Fate/Zero
4.Akemi Homura from Madoka Magika
5.Me ga Saeru no Hebi from Mekaku City Actors

1. Kaneki Ken from Tokyo Ghoul
Kaneki should really try harder to count down from 1000 in sevenths, cause his sanity is long long gone. This guy's personality splits more than his cells do, and he freaging hallucinates each of them, plus constantly hallucinating about the person whose organs were implanted inside of him turning him into a half-ghoul,a monster that eats human flesh as the only source of nutrients. Add a kakuja on top of that, and the fact many of those personalities have all kinds of insanities of their own, and you pretty much get a guy who is all sorts of crazy.
2.Jabami Yumeko from Kakegurui
This girl is a gambling addict, and if she doesn't communicate well enough just by what she risks in simply gambling, and in the fact that she continues to gamble knowing that she can insta-win most games, in spite of again, those risks including things like loosing her livelihood and free will, tons of money, having her nails clipped off, getting shot between the eyes etc... Then the animation and the voice acting in the animation will surely make you see the she's batshit crazy, and can make it spread.
3.Uryuu Ryuunosuke from Fate/Zero
Third place goes to this serial killer, whose reaction to plans to make victims hope and then despair is "cool!", who believes killing to be art based on the colorfulness of guts, and that God is a sadistic scriptwriter that indulges in being hated and being loved and therefore throws the world into chaos. And he says all of this calmly, smiling, and falls to his knees crying over how horrible it was for people to destroy his art, when that art consisteted of impaling kids like butterflies. His final moments were witnessing his own guts spill out and smile at how he "found what he was looking for all along".
4.Akemi Homura from Madoka Magika
A once noble, fragile magical girl in the clutches of the terrible fate that would befall all of them of eventually giving in to despair and become witches... only the one girl she was trying to save, travelling back in time time and time again to do so, ends up using her wish to save all of the magical girls wiping out her existence in the process. However, at the end of the third movie, Homura basically snaps out of the memory wipe and purposely DESTROYS THE PARADISE MADE BY HER BELOVED, sacrificing the good and happiness of all magical girls and potentially the bloody universe, just to have that one girl return to being, well, normal.
5.Me ga Saeru no Hebi from Mekaku City Actors
In the world this character exists in, Medusas are a kind of monster person whose snake hair is capable of granting wishes. The snakes inhabiting this hair exist BY granting those wishes. This leads the villain of the story, one of these snakes and the insane character here presented, to up and decide to formulate a plan to live forever.
So picture this young orphaned girl. This villain orchestrates a plan to make a bunch of orphans with super powers/curses, have them be adopted, possess the one adopting them, find the oprhaned girl, make friends with her, gather a few more friends, kill them one by one and finally slauhgter the remaining right in front of that girl who is, in fact, a Medusa, to have her make the wish to set time back in order to stop it from happening, but in process her memory gets wiped out, leaving only the villain to remember each round and how it ended. Then press repeat.
That's basically the villain's plan, and he does it over and over and over again. If the uncaring and insane way of acting wasn't enough, the fact that he is straight up willing to go on doing that for eternity is truly worth calling batshit crazy.
