Help Looking for character sheet codes in the style of a job application!

Hi hello, was wondering if anyone had any freebie codes lying around that resemble something like a job application, or maybe like an employee file? I'm starting up a new group in the near future and thought it might be fun to potentially have some non-traditional character sheets in the style of a job application completed from that character's pov! I'm not looking for anything complicated, but I just want something that looks nice and really conveys this idea aesthetically if possible. Ideally I would just commission someone but I am unable to do that atm :') If all else fails I'm sure I could (eventually) cobble up something myself but I definitely struggle beyond reason with making bbcode from scratch so any suggestions are very much appreciated ^^"
