Make them swallow a key. A very, very special key that has poison dripping from it, and the only way to get the antidote is to claw the key from your own stomach, ripping yourself open, and opening a very special locked box also dripping with poison that absorbs in your skin. You can't try to throw up the key, as your mouth is sewn shut, and a metal mask is drilled into the sides of your jaws just in case. If you try to open the box before you get the key, the poison from the box seeps into your skin, and extremely moves the process of killing you foward by several hours. The only way to 'save' youself is to cut yourself open, which might be more difficult if you have basically no nails at all, and find the key, which will enable you to open the locked box, and get the antidote, which will only sustain you for a few hours, because the dose is too weak. If, by some miracle, you haven't already died, then a door nearby will unlock, letting you walk around outside in a small town street, in which the nearest hospital is hundreds of miles away.
