I was think about having factions in the game that players can support. It would also giving players a reason to pvp rather than doing it just of the heck of it. Certain factions can be allies with one faction and enemies to another. Defeating a player in a hostile faction gives a player points. You can turn those points in for rewards(gear, weapons, items and faction appeal). The better the item or the higher the level the more points it'll cost. Faction gear and weapons not only tend to look cooler than normal gear, then also normally have better stats then normal store bought items to get players to fight for their faction.
For balance purposes, factions sometimes are unleave-able. Choosing to join a faction is final, so it's wise to make sure that the faction your picking is the one you are ok with spending your whole game experience with.
As for suggestions, you did mention two different churches. That can be a start for player factions.
